Discover specially evaluated Choose Well facilities that have voluntarily agreed to be scored on 11 measures of quality. The 11 Quality Measures create the framework for a facilitys Choose Well score. These were developed by stakeholders and demonstrate a facilitys 5-year rolling compliance with the State of California regulations related to a particular Quality Measure. The Choose Well application helps seniors, their families, caregiving professionals, and others locate and find placement with assisted living providers who have distinguished themselves with both their track record for compliance and their participation in the Choose Well program. Facilities within San Diego County are displayed, but only those that have volunteered to be rated will have a Facility Profile Page that displays a Choose Well Score. Periodically, the facility providers scores are changed, using the Choose Well Scoring Device, based on when the State visits the facility. Facility provider participation and willingness to receive a Choose Well Score shows their commitment to the San Diego County community.


The Choose Well Score is a computed score with a range of 1 to 100. Scores below 70 are not published. Facilities scoring a 69 or below, receive a Participant designation instead of a numerical score. Facilities that are newly licensed and do not have a public document history are shown as Newly Licensed. Once the newly licensed facility receives a visit by the State that generates a public document, their Choose Well Score will be computed. The higher the Choose Well Score, the better the facilitys compliance with State regulations or Health and Safety Code as they relate to the 11 Quality Measures.
